What is adult spastic paralysis?
In adults, spastic paralysis usually results from a cerebrovascular accident (CVA), usually expressed as spastic hemiplegia affecting ipsilateral upper and lower limbs.
The diagnosis is clear and defined. The evolution and prognosis require monitoring over time. Thus, stabilization of the lesional spasticity can be seen after a period of 6 months, during this period of time the injury is a flaccid paralysis.
The surgeon must inform the patient about evolution, perspectives and possible surgical solutions fot the posible deficits, in order to improve the patient healing expectations from the initial phase of the stroke.
What can surgery achieve?
Arms and hands
The upper extremity can be functionally reactivated if stereognosia is conserved, that means if the patient is capable of recognize objects touching them without seeing them. If cortically, hand representation has not been eliminated by the vascular cerebral lesión, then the surgical treatment can be useful in the functional reconstruction of the upper extremity.
In those cases where functional reconstruction can’t be achieve, postural treatment and deformities correction treatment is needed with the target of reduce pain, improve higiene and achieve a good esthetic result.
Lower extremities
Lower extremities have an automatic function, that in most patients, can be improved with safe and fiable surgeries.
